_PARAGRAPH JOB_DESCRIPTION.SHARE.HTML Midland, Michigan Pharmacy 8am -4:30pm Part-Time Day Shift Ambulatory Clinical Quality 46374 Job Description Summary This position provides ambulatory clinical pharmacy services (disease management and comprehensive medication review) for patients that range in age from birth to 100+ years. The purpose of ambulatory pharmacy services is to optimize the care of outpatients and achieve therapeutic outcomes in specific chronic disease states as outlined in the Collaborative Drug Therapy Management Agreements (CDTM) with providers. Responsibilities (50%)
  • Provides clinical consultation and clarification to practitioners.
Collaborates with physicians, nurses, or otheroffice health‑care professionals regarding drug information and appropriate drug therapy. Collaborates routinely with practitioners regarding medication therapy changes per current standards. Suggests appropriate, cost‑effective therapeutic alternatives to medical staff as needed. Assists physicians/office staff with any medication questions, insurance issues, hospital follow‑up visits, medication reconciliation, refills, referrals, Annual Medicare Wellness visits, etc. Provides accurate, adequate, and timely drug information to professional staff. ( 25%)
  • Provides drug information to patients and their families. Provides education to patient regarding medications and disease state. Clarifies any medication questions with any of the patient's physicians/specialists when needed. ( 25%)
  • Reads, extracts, and interprets information in patient charts appropriately.
Utilizes standard work while in patient visits and during telephone calls. Documents all clinical activities and interventions accurately and completely.
OTHER DUTIES AND RESPONSIBILITIES
C omplies with established corporate and departmental policies, procedures, objectives, quality assurance methods, and safety codes. Demonstrates compliance with licensing, regulatory and accrediting agency provisions as required. Co nsistently demonstrates appropriate and professional communication behavior toward patients, customers, and coworkers. Cooperates with Management and peers to promote an environment of teamwork and collaboration. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-disease interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of therapy, and therapeutic duplications. U tilizes disease state and medication assessment per evidence based care plans, protocols, and clinical judgment. S creens all patients for immunization status and documentation in Health Record with accuracy. L ead and participate in teaching programs of the Department and other education as related to Ambulatory Complex Disease Management. P recept and mentor
IPPE, APPE
pharmacy students, and pharmacy residents. P rovide staff development support to nurses, physicians, and other healthcare providers. P rovide presentations, publications, and other informative activities on medication‑related topics to the healthcare community and public. A ttends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy. Attends meetings as required. M aintains annual mandatory education requirements specific to position as mandated by MyMichigan Health. W ill be required to travel to different clinic locations within the health system as deemed necessary. Other duties as assigned.

EXPERIENCE TRAINING AND SKILLS
Pharmacists practicing at MyMichigan Health under a Collaborative Drug Therapy Management Agreement (CDTM) must satisfy all of the qualifications below: C ompletion of a Pharm. D. degree and current licensure in the State of Michigan required (active professional and controlled substance licenses). C ompletion of a Pharmacy Practice Residency accredited by the American Society of Health System Pharmacists (ASHP) and/or Certification from the Board of Pharmacy Specialties in a relevant specialty area (e.g., Ambulatory Care, Pharmacotherapy, Oncology). C ompletion of any MyMichigan Health on‑site training:
  • Training and competency assessment of the pharmacist occurs at a minimum yearly or more frequently, as determined by the Ambulatory Pharmacy Supervisor.
  • Quality assurance and review of services is completed at least yearly by pharmacist and physicians. Any concerns shall be immediately escalated to the Ambulatory Pharmacy Supervisor and Pharmacy Director.
  • Review the Collaborative Practice Agreement at least annually by the physicians, in conjunction with the pharmacist, and document the review.
Basic knowledge of personal computer. A nalytical skills needed to interpret clinical data and research drug information. W ritten and interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner. PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS
Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Overall vision and hearing is necessary with or without assisted device(s). Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required. Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake. Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.
