This position offers an exciting opportunity to utilize your clinical expertise, foster meaningful patient relationships, and contribute to improving health outcomes. You will be responsible for dispensing medications accurately, providing medication counseling, and supporting various health initiatives to promote wellness in the community. Your energetic approach and dedication will help ensure our pharmacy remains a trusted resource for all patients. Responsibilities Dispense prescription medications accurately while ensuring compliance with all legal and safety standards Provide comprehensive medication counseling and patient education on proper medication use, potential side effects, and interactions Conduct patient assessments to identify medication-related problems and recommend appropriate interventions Manage inventory levels effectively, including ordering, stock rotation, and compounding medications when necessary Administer immunizations and perform medical management services such as medication therapy reviews and chronic disease monitoring Utilize pharmacy software and electronic health record (EHR) systems to document patient interactions and maintain accurate records Support patient care initiatives by collaborating with healthcare providers on medication management plans, including IV infusions and aseptic techniques for sterile preparations Requirements Valid pharmacist license with current registration in the applicable jurisdiction Proven experience in LTC (Long-Term Care) pharmacy or hospital settings is highly desirable Clinical pharmacy experience with a focus on critical care, pediatrics, or acute care environments Strong knowledge of physiology, anatomy, medical terminology, and pharmacology principles Hands-on experience with pharmacy software, EMR systems, EHR systems, and inventory management tools Ability to perform compounding medications using aseptic techniques and handle IV insertions or infusions confidently Knowledge of HIPAA regulations to ensure patient privacy and confidentiality at all times Experience with immunizations administration and pain management protocols is preferred Pharmacy residency or equivalent clinical training is a plus; pharmacy technician experience is also beneficial Join us in making a difference through dedicated patient service!
