Medication Policy General Description:
Pharmacists in the Center for Medication Policy collaborate with healthcare professionals across the health system to promote safe, effective and fiscally responsible use of medications.
Position Summary:
A clinical pharmacist position in the Center for Medication Policy is immediately available. Candidates must possess excellent writing, organizational, analytic, clinical and interpersonal skills. This clinical pharmacist position has many exciting and challenging responsibilities, the major ones include: Work with pharmacy teams, healthcare providers and committees to establish policies and implement initiatives that improve medication utilization and overall quality of patient care Support activities that impact the health system formulary Lead health system formulary evaluation teams to facilitate medication/class reviews, guideline development and review Collaborate with teams to identify opportunities for medication stewardship Collaborate with teams to manage drug shortages Promote safe use of medications by collaboration with pharmacy informatics and medication safety teams Contribute to and maintain the Center for Medication Policy webpage Participate in the development and analysis of medication utilization evaluations to recommend improvements Participate in research related to medication policy and utilization to impact delivery of quality care Provide accurate and timely responses to drug information requests Contribute to student and resident education, including serving as a preceptor for APPE students Provide staff education and seek opportunities to publish Work with peers to achieve division and departmental goals Take responsibility for personal and professional growth Maintain familiarity with all departmental programs and policies, and work as integral part of the pharmacy team Other duties as assigned Minimum Qualifications Education Doctor of Pharmacy Degree OR BS Pharmacy Degree Experience With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.
