$150,000-$155,000 DOE (Sign-on bonus and relocation assistance available)Background/ Drug Requirements:
Ability to successfully complete background check, health and drug screenPharmacist Job Responsibilities:
- Review and verify medication orders for long-term care facility residents to ensure accuracy and appropriateness
- Conduct comprehensive drug regimen reviews and identify potential medication-related issues
- Collaborate with physicians, nurses, and healthcare teams to optimize patient medication therapy
- Monitor for drug interactions, contraindications, and adverse effects
- Ensure compliance with federal and state regulations governing long-term care pharmacy operations
- Provide consultant pharmacist services and recommendations to facility staff
- Participate in interdisciplinary care planning meetings and provide clinical expertise
- Maintain accurate documentation and records in accordance with regulatory requirements
- Educate healthcare staff on proper medication administration and storage procedures
- Respond to medication-related inquiries and provide drug information support
- Oversee medication storage, handling, and disposal protocols within the facility
