In this vital role, you will be responsible for ensuring the safe, effective, and efficient dispensing of medications while providing exceptional patient care. Your expertise will support clinical decision-making, medication management, and patient education within a fast-paced hospital or clinical setting. The ideal candidate is passionate about patient service, possesses strong clinical skills, and thrives in a collaborative environment committed to improving health outcomes. Responsibilities Dispense prescriptions accurately while verifying dosage, interactions, and contraindications using advanced pharmacy software systems. Provide comprehensive patient assessments to determine appropriate medication therapy and identify potential issues. Collaborate with healthcare professionals to develop and modify medication plans, including pain management, pediatrics, and critical care protocols. Administer immunizations and perform medication administration procedures following strict aseptic techniques and medical guidelines. Manage inventory levels efficiently, including compounding medications and handling IV infusions with precision. Maintain detailed documentation within Electronic Health Record (EHR) systems while adhering to HIPAA regulations to ensure patient confidentiality. Educate patients on medication usage, potential side effects, and medical terminology to promote understanding and compliance. Support pharmacy technicians in daily operations, including medication preparation and inventory management tasks. Stay current with hospital policies, acute care practices, physiology knowledge, and medical management updates to provide optimal patient care. Skills Extensive experience in LTC (Long-Term Care) pharmacy settings with a strong foundation in hospital or acute care environments. Critical care experience coupled with a pharmacy residency enhances clinical decision-making capabilities. Proficiency with pharmacy software platforms and EMR/EHR systems for accurate record keeping and medication tracking. Skilled in compounding medications, aseptic techniques, IV insertion, infusion preparation, and sterile procedures. Knowledge of anatomy, physiology, medical terminology, and pharmacology principles relevant to diverse patient populations including pediatrics. Ability to perform patient assessments effectively while delivering personalized care through immunizations and medication management. Experience with inventory management systems ensuring proper stock levels of medications including controlled substances. Strong understanding of HIPAA compliance standards to safeguard patient information at all times. Excellent communication skills for collaborating with healthcare teams and educating patients about their treatments.
