The pharmacist will be responsible for performing final verification checks, clinical documentation review, medication management activities, and ensuring compliance with all applicable pharmacy regulations and quality standards. Experience with home infusion services, sterile compounding, and clean room operations is required. Key Responsibilities Perform final verification and quality checks on compounded medications and prescriptions. Review laboratory results, clinical documentation, and patient records to support safe medication therapy. Ensure compliance with federal, state, and local pharmacy regulations. Maintain accurate and confidential patient records and pharmacy documentation. Provide medication counseling, including dosage instructions, administration techniques, storage requirements, potential side effects, and drug interactions. Monitor medication therapy and identify potential drug-related problems. Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care. Compound medications for internal and external use as required. Prepare and oversee sterile compounding activities in accordance with established standards and procedures. Supervise pharmacy technicians and support staff as appropriate. Manage pharmaceutical inventory and ensure proper storage and handling of medications and supplies. Educate patients on the use of medical devices and medication administration techniques. Participate in quality assurance, regulatory compliance, and patient safety initiatives. Required Qualifications Doctor of Pharmacy (PharmD) degree. Active Pharmacist license in good standing. Strong knowledge of pharmacy laws, regulations, and professional standards. Experience reviewing clinical documentation and medication therapy. Excellent communication and interpersonal skills. Ability to work effectively in a fast-paced healthcare environment. Required Experience Home infusion pharmacy experience. Sterile compounding experience. Clean room operations knowledge and experience. Experience performing pharmacist final verification and quality review activities. Preferred Qualifications Experience in specialty pharmacy or infusion pharmacy settings. Familiarity with accreditation and regulatory standards related to sterile compounding and home infusion services. #

$65.00 - $69.03 per hour
