Location:
Angel Fire, NM Key Responsibilities:
Review and interpret physician orders and prescriptions for accuracy and appropriateness. Collaborate with healthcare teams to optimize medication regimens and ensure patient safety. Monitor patient response to therapy and adjust medications as necessary. Provide patient counseling and education regarding medications and potential side effects. Maintain accurate and thorough documentation of pharmaceutical care activities in compliance with regulatory standards. Participate in quality improvement initiatives and medication safety programs.
Desired Experience and Qualifications:
Current licensure as a Pharmacist with clinical pharmacy experience. Strong knowledge of pharmacology, drug interactions, and therapeutic protocols. Ability to effectively communicate complex medication information to diverse audiences. Experience in medication therapy management and clinical decision-making. Proficient with pharmacy software and electronic health records. Demonstrated commitment to continuous learning and professional development.
