Full-Time Adirondack Health is seeking an experienced Oncology Pharmacist to join our Pharmacy Services team. This role is ideal for a pharmacist with strong oncology expertise and a passion for delivering high-quality cancer care.
Responsibilities:
✔ Review and optimize chemotherapy orders according to NCCN guidelines and treatment protocols ✔ Collaborate with oncologists, nurses, and healthcare providers on medication management and treatment plans ✔ Monitor chemotherapy dosing, stability, concentrations, beyond-use dating, and lifetime dose tracking ✔ Assist with chemotherapy inventory management, procurement, and waste reduction initiatives ✔ Ensure compliance with USP <797>, USP <800>, and all applicable sterile compounding regulations ✔ Support medication reconciliation and oncology treatment planning
Qualifications:
Active New York State Pharmacist License PGY2 Oncology Residency and/or BCOP Certification preferred 1-5+ years of oncology pharmacy experience, including chemotherapy preparation and regimen review Strong knowledge of oncology protocols, NCCN guidelines, and sterile compounding standards Excellent communication and interdisciplinary collaboration skills
