PrimaryOne Health is currently seeking a Clinical Pharmacist for our Columbus health centers.
Essential Job Responsibilities:
Direct Patient Care Provide Comprehensive Medication Management (CMM) for patients with chronic conditions such as diabetes, hypertension, COPD/asthma, heart failure, HIV, HCV, and behavioral health conditions. Assess the status of the patient's health problems and determine whether the prescribed medications are optimally meeting the patient's needs and goals of care. Evaluate the appropriateness and effectiveness of the patient's medications. Recognize untreated health problems that could be improved or resolved with appropriate medication therapy. Follow the patient's progress to determine the effects of the patient's medications on his or her health. Advise the patient on how to best take medications. Refer the patient to his or her physician or other health professionals to address specific health, wellness, or social services concerns as they arise. Interdisciplinary Collaboration Consult with the patient's physicians and other health care providers in selecting the medication therapy that best meets the patient's needs and contributes effectively to the overall therapy goals. Apply specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects, and drug interactions, in performing patient care activities in collaboration with other members of the health care team. Population Health & Quality Improvement Lead or support initiatives tied to UDS measures, value‑based contracts, and chronic disease management goals. Identify and manage high‑risk patient cohorts using EHR reports and registries. Administer medications per company protocol, including Vivitrol. Maintains contact with other department heads, medical staff, nursing staff and others to determine needs, resolve problems, improve processes and promote effective drug therapy. Pharmacy Operations & Compliance Ensure compliance with 340B program requirements, including prescribing practices, patient eligibility, and medication access workflows. Participation in Quality Assurance projects related to the PrimaryOne Health Pharmacy Program and the 340B program. Maintains knowledge of latest pharmacy products and all governing laws. Education & Training Provide education to providers, nurses, medical assistants, and trainees on medication therapy, guidelines, and best practices. Educate the patient on other important steps to improve or maintain health, such as exercise, diet, and preventive steps like immunization. Precept pharmacy students or residents if applicable Other duties or special projects as assigned. Successful candidates must possess a doctorate degree in Pharmacy, Registered Pharmacist and minimum two years of clinical pharmacy practice or related experience.
