The Outpatient Pharmacist at Medina Hospital supports both hospital discharge patients and the surrounding community within a collaborative, patient-focused outpatient pharmacy environment processing approximately 225 prescriptions per day. This role is responsible for filling and verifying prescriptions, answering medication-related questions, handling patient calls and inquiries, completing prior authorizations and medication access support tasks, administering vaccinations, and assisting with bedside delivery services and discharge medication coordination. The pharmacist works closely with pharmacy technicians and providers to ensure efficient workflow, medication access, and a high level of patient satisfaction and care. A caregiver in this role will work Monday through Friday with shift options from 7:45am to 4:15pm or 9:45am to 6:15pm. Preferred availability includes Tuesdays and Fridays, though flexibility may be considered based on candidate availability. This role includes 1 weekend day per quarter at Independence FHC and 1 holiday about every 4 years at the Euclid Avenue location. There are no regular float expectations at this time. A caregiver who excels in this role will: Provide pharmacy services and ensure the appropriate selection, dosing, administration and dispensing of medication orders. Provide safe, efficient, patient-centered, evidence-based medication services. Verify medication orders and/or dispense medications accurately according to state and federal regulations. Contribute to medication safety initiatives. Document services in a complete, accurate and timely manner. Ensure all pharmacy operations comply with national accreditation standards. Maintain professional interactions with patients to facilitate successful patient experiences. Educate and train students, residents, other caregivers, and/or patients. Be accountable for drug use policy adherence. Minimum qualifications for the ideal future caregiver include: PharmD or Bachelor's of Science in Pharmacy from an accredited College of Pharmacy Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire Basic Life Support (BLS) Certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations within 90 days of hire for those administering immunizations Preferred qualifications for the ideal future caregiver include: Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area Board Certification Prior outpatient pharmacy experience
Physical Requirements :
Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 20 lbs. Position often includes reaching, stretching, stooping, climbing steps and sitting. Must have normal or corrected vision. Must have ability to clearly communicate verbally by phone or in person

Pay Range Minimum Annual Salary:
$101,900.00
Maximum Annual Salary:
$155,390.00 The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate's actual compensation will be determined after taking factors into consideration such as the candidate's work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.).
