Position Summary:
Provides comprehensive pharmaceutical care through integrated drug distribution, patient counseling and teaching. Staff Pharmacists fill orders for medications, monitor patient's drug therapies and provide drug information. Staff Pharmacists supervise and direct supportive staff. In the absence of the Director of Pharmacy, a staff pharmacist may be required to assume the responsibilities and perform the duties of the Director.
Job Duties/Responsibilities:
Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete pharmacy patient medication profiles. Maintains awareness of developments in pharmacy and healthcare that relate to job responsibilities and integrates them into their practice. Supports the pharmacy's pharmacoeconomic efforts. Reviews the use of targeted medications for compliance with current pharmacoeconomic programs. Monitors drug therapy regimens for contraindications, drug-drug reactions, drug-food interactions, allergies, adverse reactions and appropriateness of drug, dosage form, regimen route, and therapeutic duplications. Assists with pharmacokinetic consult service and drug dosing regimens per hospital protocol.
