Doctor of Pharmacy Licensed Pharmacist Full Job Description Full Time Job Summary:
Provides patient care by preparing and dispensing medications and providing inpatient and outpatient counseling for the purpose of ensuring proper drug utilization.
Minimum Education:
Completion of a Doctorate of Pharmacy Degree from an accredited school of Pharmacy. Licensure, Registration and/or
Certification:
Possession of Oklahoma licensure within the time limits pursuant to the Residency Program.
Work Experience:
0 - 6 months related experience.
Knowledge, Skills and Abilities:
Working knowledge of clinical pharmacology and drug interactions. Working knowledge of presentation and word processing software. Ability to organize and prioritize work in an effective and efficient manner. Ability to be detail oriented as required in the examination of clinical and numerical data.
Essential Functions and Responsibilities:
Identify opportunities for improvement of the organization's medication- use system. Design and implement quality improvement changes to the organization's medication-use system. Prepare/dispense medications following existing standards of practice, and the organization's policies and procedures. Perform all necessary aspects of the medication-use system. Use information technology to make decisions and reduce error. Collect and analyze patient information. When necessary, make and follow up on patient referrals. Design evidence-based therapeutic regimens. Design, recommend, communicate, and implement evidence-based monitoring plans. Provide timely responses to requests for drug information from patients and health care providers. Establish collaborative professional relationships with members of the health care team and professional pharmacist-patient relationships. Provide effective medication and practice-related education, training, or counseling to patients, caregivers, health care professionals, and the public. Contribute to departmental leadership and management activities. Conduct a practice-related project using effective project management skills.
Decision Making:
Independent judgment in planning the sequence of operations and making minor decisions in a complex technical and professional field.
Working Relationships:
Works directly with patients and/or customers. Works with internal customers via telephone or face to face interaction. Works with other healthcare professionals and staff.
