Job Description

Inpatient Pharmacist -
Contract Location:

Hours Per Week Responsibilities:
Verify and process inpatient medication orders using EPIC. Provide clinical evaluation of medication therapies for hospitalized patients. Ensure accurate and timely dispensing of medications. Monitor medication therapy effectiveness and maintain safe medication practices. Support inpatient care and medication management activities. Provide overnight on-call pharmacy support as required. Collaborate with physicians, nurses, and healthcare teams. Conduct medication reconciliation and support discharge planning. Respond to clinical questions and provide drug information. Ensure compliance with hospital policies, pharmacy regulations, and safety standards. Participate in weekend coverage as scheduled. Document clinical interventions and pharmacy activities in EPIC. Contribute to medication safety and quality improvement initiatives.
Preferred:
Inpatient hospital pharmacy experience. Familiarity with EPIC electronic medical record system.
Requirements:
Doctor of Pharmacy (PharmD) required. Active Oregon Pharmacist License required. Strong clinical knowledge of acute care pharmacotherapy. Experience working in fast-paced hospital environments.
