Licensed Pharmacist Full Job Description Outpatient Infusion Pharmacist Location:

Weeks Requirements:
Active Oregon Pharmacist License required Minimum 1 year of pharmacist experience First-time travelers accepted Outpatient Infusion Pharmacy experience required Strong knowledge of medication preparation, dispensing, and monitoring practices Experience with Epic EMR preferred
Responsibilities:
Review, verify, and dispense medications in accordance with physician orders and regulatory requirements Prepare and oversee infusion therapies while ensuring medication safety and accuracy Monitor patient medication regimens for effectiveness, interactions, and adverse reactions Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes Provide medication counseling and education to patients and healthcare staff as needed Maintain accurate documentation and compliance with pharmacy policies and standards Participate in quality improvement and medication safety initiatives
Assignment Details:
No floating required No on-call requirements No weekend requirements Block scheduling available Expected workload ratio: 1:20
Preferred Qualifications:
