Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A hybrid clinical and staff pharmacist position is available in State College, PA, offering an exciting opportunity to contribute to both patient care and pharmacy operations. This role involves dedicating approximately 60% of time to clinical work and 40% to staffing responsibilities, focusing on managing medication regimens within a Population Health and Ambulatory Care setting, with an emphasis on chronic disease states such as diabetes.

Key Responsibilities:

Provide clinical pharmacy services by managing and optimizing patient medication regimens in collaboration with the healthcare team. Support ambulatory care and population health initiatives to improve patient outcomes. Perform staffing duties including prescription verification and dispensing during assigned shifts. Collaborate with Physicians and nursing staff to implement and expand clinical pharmacy programs. Participate in the development and growth of clinical services within the health system.

Qualifications:

Active Pennsylvania pharmacist license is required. Experience in ambulatory care or inpatient pharmacy settings preferred. Completion of a residency program is highly desirable. Strong clinical knowledge with a passion for patient-centered care. Proficiency in Meditech or similar pharmacy information systems is beneficial.

Schedule:

Variable day and afternoon shifts. Primarily 8-hour shifts.
