Job Description

Join our team as a Full-Time Clinical Pharmacist Specialist! Clinical Pharmacists provide safe, effective, and patient‑centered medication management for specialized patient populations. They work collaboratively with physicians, nurses, and interdisciplinary care teams to optimize medication therapy, support medication‑use systems, and improve quality, safety, and outcomes. Clinical Pharmacists deliver direct patient care while supporting pharmacy operations, education, research, and practice advancement initiatives.
Primary Responsibilities:
+ Provide comprehensive, evidence‑based medication management and direct patient care + Collaborate with multidisciplinary teams to optimize therapy, prevent adverse drug events, and improve patient outcomes + Identify high‑risk patients and implement safety and quality interventions + Participate in stewardship programs, restricted medication initiatives, and medication‑use optimization + Conduct medication reviews, reconciliation, discharge counseling, and ongoing patient monitoring + Support pharmacy operations, including decentralized services, dispensing, and specialty medications as assigned + Lead or support clinical programs, service lines, and therapeutic initiatives + Develop and evaluate policies, procedures, and quality improvement projects + Participate in research, practice advancement, and regulatory or compliance initiatives + Serve as a clinical resource, educator, and preceptor for students, residents, and staff + Graduate of an ACPE accredited College of Pharmacy + B.S. degree in Pharmacy Doctor of Pharmacy (PharmD) degree; PharmD preferred. + Current licensure as a Pharmacist in practicing state required + Completion of ASHP accredited PGY-1 Pharmacy Residency or 3 years clinical pharmacy experience + Knowledge and expertise in advanced pharmacy practice and service, comprehensive medication management, and evidence-based medicine in specialized area appropriate for clinical assignments + BPS Board Certification preferred.
