Job Description
Pharmacist- PRN Who We Are Conemaugh Nason Medical Center is a 45-bed hospital that has served rural and suburban populations in Roaring Spring, Altoona, and parts of Blair, Bedford, and Huntingdon counties for 121 years. With a state-of-the art cardiac catheterization lab, Conemaugh Nason offers a range of inpatient and outpatient cardiac services close to home. The hospital provides 24/7 emergency care as an accredited chest pain center, and offers comprehensive outpatient, imaging, diagnostic, and specialty services. How you'll contribute Coordinates and provides pharmaceutical care for patients, collaborating with the interprofessional team to optimize safe, effective medication use. Reviews and verifies orders, manages drug therapy protocols, educates staff/patients, and supports quality and regulatory standards. Reports to the Director or Manager of Department. Education:
Doctor of Pharmacy.License:
Current license with the state Board of Pharmacy.Certifications:
BCLS required; ACLS preferred.Skills:
Critical thinking, decisive judgment, and ability to work with minimal supervision in stressful environments. Rapid learning and navigation of multiple pharmacy/clinical software systems. Effective English communication (read, write, speak, and understand). Ability to complete the Skills Competency Checklist within 90 days of hire.Essential Functions:
Manages drug therapy protocols as part of an interdisciplinary team. Reviews, interprets, and evaluates medication orders to identify, prevent, and resolve drug-related problems. Provides drug information and education to medical staff, healthcare professionals, patients, and pharmacy students. Documents interventions and outcomes for drug therapy quality improvement. Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software. Manage patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling. Participates in performance improvement and required continuing education. Performs other duties as assigned.Additional Information Patient Populations Served:
