Job Description

Position Summary:
The Population Health Pharmacist is responsible for improving the health of populations through optimized medication management, evidenced based prevention strategies, and promoting cost effective care. The Population Health Pharmacist is embedded within a multidisciplinary clinical team, works under a collaborative practice agreement with the physician(s), and provides telephonic and/or virtual interventions.
Education, License & Cert:
Graduate of an ACPE-accredited School of Pharmacy with a Doctor of Pharmacy (Pharm.

D.) degree. Current Licensure as a pharmacist in the state of New York and in the state of Pennsylvania. Completion of a pharmacy residency program (PGY1) or equivalent clinical experience. Board certification in ambulatory care pharmacy (BCACP) or other relevant certification preferred.
Experience:
A Population Health Pharmacist should personify strong clinical knowledge and experience in medication therapy management, excellent communication and interpersonal skills and the ability to work independently and as part of a multidisciplinary team. They should also be able to show proficiency in using electronic health records (EHR) and other healthcare technology.
Essential Functions:
Analyze data from various sources to identify medication-related trends, quality gaps, and areas for improvement Develop and implement strategies to optimize medication use, adherence to medications, control costs, and improve quality metrics like HEDIS and STAR ratings Support value-based care initiatives by focusing on areas that align with value-based care metrics, such as reducing hospital readmissions, improving chronic disease management, and closing care gaps Attend, participate, and present pharmacy related initiatives and performance at Population Health meetings, including payer collaboratives and Joint Operating Committees Assess each of the patient's medications for indications, effectiveness, safety, tolerability, and adherence issues and provides care in a way that maximizes patient satisfaction Build relationships with providers to create a feedback loop between pharmacy and clinical teams. This may include providing consultation to providers on prescribing practices and promoting best practices like generic prescribing and deprescribing Titrate/manage/monitor the patient's medication therapy to optimize clinical outcomes and will identify and address gaps in care Demonstrates proficiency in medication and disease management, as well as thorough understanding of the medications, clinical conditions, and current treatment guidelines for the conditions in their assigned clinical areas Assess patient's medication therapy regimen for appropriateness, effectiveness, tolerability, safety, and potential for adverse events Gathers appropriate clinical, imaging, and laboratory data necessary to make appropriate decisions on the management of the patient's medication therapy and related diseases and conditions Makes clinically appropriate changes to regimen which will maximize effectiveness and patient satisfaction, while working to minimize potential for both acute and chronic complications. Provides in-services, in-depth drug information, and other educational and informational materials to members of the medical, nursing, and other healthcare professionals Provides patients with appropriate educational materials and information to ensure they understand and actively participate in their care Provides appropriate documentation of each encounter in the electronic health record and conveys that information to the referring physician and next provider of care as appropriate
