Job Description

Category Pharmacy - Clinical Pharmacist Job Type Direct Hire This opportunity combines direct patient care with frontline pharmacy operations in a hybrid clinical and staffing pharmacist role. The position involves approximately 60% clinical duties focused on Population Health and Ambulatory Care, with a strong emphasis on managing medication regimens for patients with chronic diseases such as diabetes. The remaining 40% of time will be staffing-related, supporting daily pharmacy operations within the health system. The health system is dedicated to expanding its clinical pharmacy services, making this an ideal position for a motivated clinical pharmacist who is passionate about patient care and eager to help grow clinical programs. As the program develops, there is potential to transition into a fully clinical role.

University Park area,
State College, PA Schedule:
Primarily day shifts with variable days and afternoons; 8-hour shifts
Technology:
Meditech electronic health records system
Key Responsibilities:
Conduct clinical medication reviews and patient counseling, focusing on chronic disease management Collaborate with healthcare teams to optimize medication therapy and improve population health outcomes Provide operational support through staffing duties when not engaged in clinical services Assist in expanding the clinical pharmacy program as directed by the Department of Pharmacy leadership
Qualifications and Desired Experience:
