Job Summary:
This Clinical Ambulatory Pharmacist will support the pulmonary specialty. The pharmacist will provide direct patient care through collaborative practice agreements and collaboration with providers and care team members. The pharmacist will provide disease state management for a range of pulmonary disease states, including asthma, COPD, cystic fibrosis, and smoking cessation. This pharmacist will serve as a medication resource, will help with the optimization of patients' medication regimens, assist with population health initiatives, and may have other responsibilities, as assigned. In this role, the pharmacist will provide direct patient care via in-person office visits, virtual visits, and telephone encounters. This is a hybrid position and may require the pharmacist to travel between clinics. Using independent decision making and critical thinking skills, the pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The pharmacist ensures patient care is maintained through compliance with the department's quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services.
EXPERIENCE DESCRIPTION
A minimum of two years of experience in an Ambulatory setting is required.
EDUCATION DESCRIPTION
Bachelor's or Pharm. D. degree from an ACPE certified school of pharmacy is required. PGY1 residency is required. PGY2 ambulatory residency or equivalent experience is required.
SPECIAL SKILLS DESCRIPTION
Demonstrates effective use of computers and applications (e.g., MS Word, Excel, Outlook) as appropriate to practice and communicate effectively.
LICENSURE DESCRIPTION
Active Wisconsin Pharmacist License is required or eligible for and must obtain within 3 months.
