Where You'll Work Welcome to Saint Joseph East, a 217-bed, full-service, community hospital located in a convenient setting in Lexington, Kentucky. Saint Joseph East also includes the Women's Hospital at Saint Joseph East which is dedicated to supporting the well-being of women in Central and Eastern Kentucky and serves a 16-bed Level III Neonatal Intensive Care Unit (NICU). Job Summary and Responsibilities
JOB SUMMARY / PURPOSE
The Clinical Pharmacist Ambulatory Services provides direct patient care in hospital or community-basedclinics. The pharmacist works collaboratively with physicians, nurses, other healthcare providers, andpatients to optimize drug therapy and outcomes using evidence-based standards of care. This job isresponsible for providing effective, efficient and accurate ambulatory pharmaceutical care to achieveorganizational, patient care, and quality measure goals and objectives. The incumbent is responsible forworking within the scope of their license, performed in accordance with organizational protocols,internal guidelines/standards, and any applicable State regulatory requirements.
ESSENTIAL KEY JOB RESPONSIBILITIES 1.
Participates in the evaluation and management of diseases and health conditions in collaboration withother health care providers2. Prescribe pharmaceutical agents as outline in the approved collaborative drug therapy managementagreement including initiating, adjusting, and discontinuing drug therapy3. Order and interpret drug therapy-related tests4. Ensure patients have access to and receive safe, efficacious and cost-effective medication therapy5. Work collaboratively to resolve medication access and adherence issues 6. Address medication-related issues to support care coordination and transitions of care7. Performs appropriate Point-of-care laboratory testing as required for the visit8. Provides patients and/or families with focused teaching on safe and effective use of prescribedmedications and non-pharmacologic management of chronic health conditions9. Precepts Pharmacy Interns and Pharmacy Residents. May serve as the Residency Program Director orCoordinator if applicable to their location.

MINIMUM QUALIFICATIONS
Required Education and Experience Graduation from an accredited school of pharmacy
Preferred Education and Experience:
Pharm.

with:
  • Completion of three (3) years of practice experience with atleast 50% of time spent in ambulatory care activities, or
  • Completion of a PGY2 Ambulatory Care Pharmacy residency, or
  • Completion of a PGY1 residency plus one (1) year of practiceexperience with at least 50% of time spent in ambulatory careactivities
Required Licensure and Certifications:
Active Pharmacist License in good standing in the state(s) in which the employee works.
PHARM:
KY Required Minimum Knowledge, Skills,Abilities and Training:
  • Current knowledge of evidence based clinical pharmacypractice and pharmacotherapies
  • Knowledge of Health Plans such as Medicare, Medicaid, andcommercial plans
  • Demonstrates knowledge and skills necessary to provide careappropriate to all age groups of patients served and infectioncontrol, safety and risk management standards as relates to theambulatory pharmacy setting.
