Position Summary This position will provide comprehensive clinical pharmacy services within the ambulatory care clinics (ACTIVE Care Clinic), focusing on the management of chronic disease states in collaboration with physicians and multidisciplinary care teams. The clinical pharmacist will manage primary care and specialty patients through evidence-based medication management, patient education, and protocol-driven care. The ideal candidate will demonstrate strong clinical judgment, the ability to work independently with minimal supervision, and effective collaboration within a multidisciplinary setting. Essential Functions and Responsibilities of the Job List of broad job specific functions. 1. Work in the multidisciplinary ACTIVE Care Clinic providing direct patient care services 2. Manage chronic disease states including diabetes, hypertension, anticoagulation, and heart failure through protocols approved by Best Practice Committee 3. Conduct comprehensive medication management, medication reconciliation, and medication optimization 4. Monitor patient outcomes and adjust therapy based on clinical guidelines, laboratory values, and patient-specific factors 5. Provide patient education and counseling regarding medications, disease states, lifestyle modifications, and adherence strategies 6. Collaborate closely with physicians, case managers and other healthcare professionals to optimize patient care 7. Document clinical interventions and patient encounters accurately and timely within the electronic health record 8. Identify and escalate clinical concerns appropriately to providers and leadership when needed 9. Participate in development, implementation, and continuous improvement of clinical workflows and protocols 10. Support quality improvement initiatives and organizational goals related to patient outcomes, productivity, and growth 11. Maintain current knowledge of evidence-based guidelines and best practices related to ambulatory care pharmacy services 12. Possess strong analytic and problem-solving skills 13. Ability to be at work and be on time 14. Ability to follow company policies, procedures, and directives 15. Ability to interact in a positive and constructive manner 16. Ability to prioritize and multitask 17. Other related duties and projects as assigned Specific job related responsibilities and duties: Provide direct pharmacy care services through interaction with patients within the ambulatory care clinics Manage and monitor patients with chronic disease states including diabetes, hypertension, anticoagulation, and heart failure Optimize medication therapy utilizing evidence-based guidelines and approved protocols Collaborate with providers and multidisciplinary teams to improve clinical outcomes and patient care coordination Educate patients on disease state management, medications, adherence, and lifestyle modifications Document interventions, recommendations, and patient encounters accurately and timely Participate in workflow optimization and continuous quality improvement initiatives Assist with implementation and maintenance of clinical protocols and procedures Support organizational goals related to patient access, productivity, quality metrics, and growth Other duties as assigned •Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications.
