Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A hybrid clinical/staff pharmacist role is available at a respected medical center in State College, PA. This position offers a dynamic balance, with approximately 60% of the time dedicated to clinical work focusing on Population Health and Ambulatory Care, and 40% devoted to staffing responsibilities. The clinical focus will be on managing medication regimens for patients within the health system, especially targeting chronic conditions such as diabetes. This opportunity allows for growth into a fully clinical role as the program expands, making it ideal for a pharmacist passionate about patient care and eager to contribute to the development of clinical services. The department leadership is committed to expanding clinical pharmacy offerings, creating a supportive environment for professional growth.
Key Responsibilities:
Manage patient medication regimens in ambulatory care settings with a focus on population health. Provide clinical pharmacy services to support patient outcomes. Participate in staffing duties as part of a hybrid role. Collaborate with healthcare teams to optimize medication therapy.
Qualifications:
Active Pennsylvania pharmacist license. Experience in ambulatory care or inpatient settings preferred. Completion of residency program is a strong advantage. Comfortable working with Meditech electronic health records. Strong clinical skills and passion for patient-centered care.
Schedule:
Variable days, primarily day shifts. 8-hour shifts.
