Responsibilities:

• Provide clinical pharmacy services focused on Population Health and Ambulatory Care.
• Collaborate with healthcare teams to optimize medication management for patients.
• Balance clinical duties with staffing responsibilities, dedicating approximately 60% of time to clinical work and 40% to staffing activities.
• Assist in expanding clinical pharmacy programs as the department grows.
• Utilize Meditech for patient records and medication management.
• Work variable day and afternoon shifts, primarily 8-hour day shifts.

Qualifications:

• Current Pennsylvania pharmacist license required.
• Experience in ambulatory care or inpatient pharmacy preferred.
• Completion of residency highly desirable.
• Strong passion for patient care and clinical program development.
• Excellent communication and teamwork skills.
