Category Pharmacy - Clinical Pharmacist Job Type Direct Hire This position offers a unique blend of clinical and staffing responsibilities, ideal for a pharmacist who thrives in a dynamic healthcare environment. The role involves spending approximately 60% of time on clinical work focused on Population Health and Ambulatory Care, particularly managing medication regimens for patients with targeted disease states such as diabetes. The remaining 40% of the time will be dedicated to traditional staffing duties. The healthcare facility is committed to expanding its clinical services, presenting an excellent opportunity for a pharmacist passionate about patient care and eager to help grow a clinical program. With the potential to transition into a fully clinical role as the program develops, this position offers both stability and professional growth.
Responsibilities:
Manage and optimize medication therapy for patients within the health system. Focus on ambulatory care with a special emphasis on chronic disease management. Collaborate with the healthcare team to improve patient outcomes. Balance clinical duties with staffing activities supporting pharmacy operations. Utilize Meditech for clinical documentation and workflow.
Qualifications:
Licensed Pharmacist in Pennsylvania. Experience in ambulatory care or inpatient settings preferred. Completion of a residency program is a significant advantage. Strong interest in population health and chronic disease management.
Location:
State College, Pennsylvania Hybrid work model: primarily day shifts with variable days/afternoons on 8-hour shifts. This is a direct hire opportunity with potential for temp-to-hire arrangements.
