Requirements:

• Active SC License or willing to obtain
• BC/BE (If BE, within 5 years of eligibility)
• DEA/CSR (must be obtained prior to start)​
• Clean Record
• Completed Fellowship
• Most recent NPDB Self Query Report (within 30 days active)
