Job Description Summary The Ambulatory Clinical Pharmacist provides direct patient-centered clinical services, including Medicare Annual Wellness Visits (AWVs), comprehensive medication reviews, chronic disease management, and preventive care services. This role works collaboratively with physicians, advanced practice providers, and care teams to identify medication-related issues, support quality outcomes, and optimize patient safety and health. The pharmacist documents care in the electronic health record and ensures compliance with clinical, regulatory, and quality standards while delivering high-quality, evidence-based care. Additional Job Description Minimum Training and Education:
Doctor of Pharmacy or a Bachelor's degree in pharmacy. Completion of a pharmacy residency preferred.
Required Licensure, Certifications, Registrations:
Licensed as a Registered Pharmacist by the South Carolina Board of Pharmacy.
