This a culture of winning. At Talented Medical Solutions we breed winners. Responsibilities Provide inpatient pharmaceutical care and ensure safe, effective med use for patients. Review and verify medication orders for accuracy, dosing, and interactions. Ensure compliance with regulatory standards, policies, and accreditation requirements. Collaborate with healthcare teams to optimize therapy and achieve treatment targets. Monitor patient outcomes and document medication-related interventions. Utilize performance metrics and pipelines to inform daily workflow and targets. Provide patient and family counseling on medications, adherence, and side effects. Identify opportunities to improve pharmacy processes and efficacy within the unit. Assist in formulary management and evidence-based selection of inpatient meds. Participate in daily huddles and maintain visibility of clinical activities. Support ongoing professional development and seek opportunities for advancement. Experience/Qualifications Active Pharmacist license in the relevant jurisdiction with inpatient experience. Inpatient medication order review for accuracy, dosing, interactions, and contraindications. Knowledge of regulatory standards, accreditation requirements, and compliance. Collaborate with multidisciplinary teams to optimize therapy and treatment targets. Utilize performance metrics and pipelines to guide daily workflow and targets. Monitor and document patient outcomes; track medication-related interventions. Provide patient and family counseling on medications, adherence, and side effects. Assist formulary management and evidence-based selection of inpatient meds. Participate in daily huddles; maintain visibility of clinical activities.
