EMERGENCY DEPARTMENT PHARMACIST
( Must have Completed an ASHP accredited PGY-1 Pharmacy Practice Residency to apply No Exceptions! ) Work collaboratively with multidisciplinary teams to optimize drug therapy. Provide direct patient care, including treatment assessment and medication management activities. Clinically evaluate all patients in specified area on a daily basis with relevant documentation placed in the medical record recommendations, and Interpret medication orders, profile orders and oversee the packaging, labeling and distribution of medications. Perform drug therapy monitoring and intervene as necessary on drug dosing, drug administration, drug interactions, side- effects and monitoring issues; complete therapeutic drug monitoring consults and adverse drug reaction reports. Provide accurate and timely drug information to physicians, nurses and other health professionals. Respond to level 1 trauma, code blue massive blood transfusion emergencies. Provide direct patient care and medication therapy monitoring, including antibiotic stewardship, pharmacokinetic drug dosing, renal and hepatic dosing, and patient education. Develop, implement, and monitors clinical protocols. Provides consultative services and clinical expertise to evaluate and improve medication usage and therapeutic outcomes. Process orders and prescriptions to provide medications and pharmaceutical supplies ordered by physicians and authorized medical practitioners. Facilitate discharge planning and smooth transition to alternate sites of care from a pharmacy perspective. Maintain and updates a patient profile with demographics, diagnosis, allergies, and current medications. Discuss medication order clarifications with the prescriber, document clinical interventions and cost avoidance as part of the departmental documentation program. Educate patients on discharge medications. Provide cost-effective management of drug utilization within the clinic. Monitor process and outcome of patient care relating to drug therapy. Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects. Qualifications Minimum of one year of pharmacy experience. Previous inpatient experience is preferred. Board Certification Preferred but not required. Bachelor's degree in pharmacy. Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
Licensure/Registration:
Current, valid, full and unrestricted license in one of the 50 states and/or the District of Columbia, the Commonwealth of Puerto Rico, or territories of the United States. Certification-BLS American Heart Association Advanced Cardiac Life Support (ACLS) - Emergency Department Pharmacists only Or Degree-Doctor of Pharmacy degree (Pharm.

: A minimum of one-year pharmacy experience in Emergency Medicine or Critical Care within the last 3 years. Completion of an ASHP accredited PGY-1 Pharmacy Practice Residency or equivalent experience is required . Advanced Cardiac Life Support (ACLS) is required and should be maintained through the life of the contract. Board certification is preferred.
