Job Description

UVA Physicians Group is seeking an Ambulatory Care Clinical Pharmacist for two of our primary care practice locations, UVA Primary Care Culpeper in Culpeper, VA and UVA Primary Care Locust Grove in Locust Grove, VA. The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record. Additional Responsibilities Provides clinical care to patients. Supports and enhances the quality of care in ambulatory practice areas. Enhances patient access to affordable care. Supports the educational and professional development of healthcare professionals. Engages in professional self-development. In addition to the above job responsibilities, other duties may be assigned.
MINIMUM REQUIREMENTS
Education:
Bachelor's in Pharmacy with experience or Pharm.

D. from an ACPE accredited College of Pharmacy. Degree required.
Experience:
PGY-1 residency required or equivalent clinical experience. Ambulatory care experience preferred.
License/Certification:
Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required. Board Certification or equivalent certification specific to clinical area highly encouraged.
PHYSICAL DEMANDS
Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs.
