Job Description

Category Pharmacy - Pharmacist Job Type Contract A rewarding opportunity is available for a Pharmacist to support patient care in a Long-Term Acute Care (LTAC) setting. This role involves working in a small hospital environment where your expertise will directly impact the health and recovery of patients requiring extended specialized care.
Location:
Madison Heights, VA Schedule:
Monday through Friday Day shift
Key Responsibilities:
Accurately dispense medications, ensuring compliance with all regulations and hospital policies Collaborate with healthcare professionals to review and optimize medication therapy plans Provide patient counseling and support to promote safe and effective medication use Monitor inventory and manage pharmaceutical supplies for the LTAC facility Participate in medication reconciliation and clinical rounds as needed
Desired Experience:
Licensed Pharmacist with experience in hospital or LTAC pharmacy settings preferred Strong knowledge of medication therapies used in long-term acute care Detail-oriented with excellent communication and problem-solving abilities Ability to work independently while being a proactive member of the healthcare team
