Job Description
Job Summary We are seeking a dedicated and highly skilled Clinical Oncology Pharmacist to join our healthcare team and supervise oncology staff within the cancer center. The ideal candidate will provide supervision and direction to pharmacists, tech and support personnel. This candidate possess extensive experience in clinical pharmacy practice, with a focus on oncology treatments, patient care, and medication management within a hospital setting. As a Clinical Oncology Pharmacist, you will be responsible for providing expert pharmaceutical care to oncology patients, collaborating with multidisciplinary teams, and ensuring the safe and effective use of chemotherapeutic and supportive medications. Responsibilities 1. Provides comprehensive pharmacy care and oversight of specialty. Reviews patient drug therapy and care plan with medical staff and other health care professionals. Provides pharmacokinetic dosing and other drug therapy consultations. Manages the pharmaceutical care of specific patients. 2. Develops and assesses the clinical pharmacy program/service. Develops new clinical programs and services and evaluates/improves existing Develops and conducts evaluation of programs/service related to performance improvement resource management, clinical and cost effectiveness/quality, prevention and detection of adverse drug events, benchmarking, etc. Develops methods for measuring, monitoring, and interpreting clinical, economic and other outcomes; medication use evaluations; target medication-intervention programs. 3. Serves as the Pharmacy departmental liaison for pharmacy, medical, nursing and other health care. 4. Serves as the educational resource for the organization within. Provides in-service trainings for pharmacy, medical, nursing, and other health care professional staff. Addresses the learning needs of the staff in response to emerging drug therapies and new uses of existing drug products. Tracks drugs in development and those under review by the DFA, assessing potential impact on the organization. 5. Leads or participates in organizational initiatives that optimize drug therapy Identifies drug-related problems and develops practice guidelines, protocols or procedures. Participates in the development of clinical pathways/guidelines. 6. Serves as a preceptor for students rotating through specialty practice. 7. The pharmacist responsible for the day-to-day oversight of the operation of the pharmacy. This includes but is not limited to: managing staff, ensuring compliance to all applicable state and federal guidelines, and maintaining the inventory of all medications. 2. Performs other duties as required or assigned Qualifications Graduate of an ACPE - accredited School of Pharmacy. 1 year of combined clinical pharmacy/specialty and/or residency experience required. Pharm D preferred. Must have demonstrated ability to work independently with minimal direct supervision, with the ability to set priorities and make critical decisions. Effective oral and written communication skills, as well as demonstrated interpersonal and team skills essential. Required Licensure/Certifications Must have a current, unrestricted pharmacist licensed to practice in the state of VT Board Certification in Specialty Pharmacy (CSP) within 3 years of hire for Clinical Pharmacists Leads/Supervisors in the Specialty Pharmacy BLS Certification within 30 days of hire for Clinical Pharmacists, if required to administer immunizationsPay:
