Category Pharmacy - Clinical Pharmacist Job Type Contract To Hire This clinical infusion pharmacist role offers the opportunity to work in a home infusion setting, providing critical support for patients receiving complex outpatient therapies. Based in Seattle, WA, this contract-to-hire position starts immediately with a 13-week initial term. Responsibilities include reviewing and verifying medication orders to ensure accuracy, appropriateness, and adherence to regulatory standards. You will provide clinical oversight for infusion therapies such as specialty medications, biologics, antimicrobials, total parenteral nutrition (TPN), and compounded sterile preparations. Assessing patient-specific therapy plans and monitoring outcomes to optimize effectiveness is a key focus. Collaboration with physicians, nurses, and other healthcare professionals is essential for effective medication management and therapy recommendations. Compliance with state and federal pharmacy regulations, accreditation standards, and organizational policies must be maintained at all times. Ideal candidates are local to Seattle and hold a clear, active Washington State pharmacist license. A minimum of two years of clinical experience is required, with home infusion experience highly preferred. Strong clinical background—ranging from inpatient care to IV therapies, TPN, vancomycin monitoring, biologics, and sterile compounding—is necessary. Specialty experience working with rare disease states and complex patients is a significant plus.
