WORK SCHEDULE
Full-Time / 40 hours per week Day Shifts
POSITION HIGHLIGHTS
Perform professional pharmaceutical dispensing and clinical functions in the Ambulatory Pharmacies. Provide drug information and education to patients and family members, providers, nurses and other health-care professionals.
PRIMARY JOB RESPONSIBILITIES
Conduct a thorough patient profile review to assess the appropriateness of the prescribed medication including monitoring for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received; document relevant information in medical record as required. Promote rational, cost-effective drug therapy by promoting the use of formulary items and optimizing patient care through appropriate selection of treatment options. Evaluate all provider orders and verify correctness of entry in the pharmacy computer system and computerized patient profiles. Direct pharmacy technicians in all areas of technical responsibilities. Ensure proper preparation and labeling of all medications. Ensure that controlled substances are properly stored, dispensed, recorded, and inventoried. Interview patients and provide education regarding their medications. Ensure that all bulk packaged items are properly prepared, labeled and documented. Ensure correct aseptic preparation, labeling and delivery of compounded sterile products. Perform routine visual observation and oversight to ensure proper garbing, handwashing, cleaning and aseptic technique processes is followed during sterile compounding. Provide drug information to patients, physicians, nurses, and other members of the health care team. Provide in-services to staff as requested.
REQUIRED POSITION QUALIFICATIONS
One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.
