What You'll Do:

As a Clinical Pharmacist and key member of our primary care team, you will play a vital role in optimizing patient outcomes through direct care and provider collaboration: Collaborate with providers to deliver medication reconciliation, pain management, and chronic disease support Counsel patients on safe, effective medication use

• both in clinic and at community outreach locations Monitor therapeutic progress and document care in the electronic health records (EHR) system Provide education for medical residents, providers, and the clinical team Support programs like diabetes care, tobacco cessation, and anticoagulation therapy Research and recommend evidence-based formulary changes to enhance access and care

What You Bring:

Doctor of Pharmacy (PharmD) and current WA State Pharmacist license and must meet one of the below requirements: Completion of an ambulatory practice pharmacy residency program
