Role:
Join Skilled Nursing Pharmacy as a Pharmacist Consultant, where you will play a pivotal role in enhancing patient care through expert medication management. We are seeking a passionate professional who thrives in a collaborative environment and is dedicated to improving the health outcomes of our clients.
Responsibilities:
Conduct comprehensive medication reviews and assessments for skilled nursing facilities. Provide expert consultation to healthcare teams on medication therapy management. Develop and implement individualized care plans for residents. Monitor patient outcomes and adjust medication regimens as necessary. Educate staff and patients on medication safety and efficacy. Participate in interdisciplinary team meetings to discuss patient care strategies. Stay current with pharmacy laws and regulations to ensure compliance. Build strong relationships with clients and healthcare providers to foster collaboration.
Requirements:
PharmD degree from an accredited pharmacy school. Current Washington State pharmacist license in good standing. Minimum of 2 years of experience in a clinical or consulting pharmacy role. Strong knowledge of pharmacotherapy and medication management. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Detail-oriented with strong analytical skills. Commitment to ongoing professional development and learning.
