Verifies accuracy and approprieness of medicion orders and prescriptions during order finishing. Consults with medical center staff when there are questions concerning therapeutic or policy mters regarding outpient prescriptions or inpient physician orders. Ensures formulary guidelines are followed in accordance with all applicable rules, regulions, and guidelines. Enures the accuracy of drug dispensing based on pient history and medicion profile review, good clinical practice, and other pertinent factors. Provides drug informion to physicians, nurses, and other associed health care professionals. Provides oral and written drug informion to inpient and outpient veterans, as approprie. Conducts clinical screening procedures such as the calculion of creinine clearances for " risk" pients. Provides discharge medicion counseling to veterans and family discharging to home from medicine service and short-stay rehabilition. Assists the Chief of Pharmacy in maintenance of the formulary. Ghers informion supporting the inclusion of drugs into (or removal of drug from) the formulary the nional, Veterans Integred Service Network, or local level. Participes in interdisciplinary rounds (eg: Discharge planning, falls assessments, etc.) with the interdisciplinary team and makes suggestions to improve therapy and/or ease transition to home, as approprie. Assists with instruction of professional or technical level pharmacy students and assists in the training of new technicians and pharmacists. Participes in quality improvement activities involving drug therapy and clinical pharmacy functions. Provides a monitoring program for all adverse drug reactions (as required by The Joint Commission).Work Schedule :
Full time, Monday- Friday, 8:00am
- 4:30pm. In addition, Clinical Pharmacists part of regular rotion staffing Inpient Pharmacy on Sundays (7:00am-11:00am) and holidays (7:00am-3:30pm). Typical frequency for each is 1 Sunday shift every 2 months on average and 1 holiday shift per calendar year.

Basic Requirements :
Citizen of the United Stes . (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) Educion Gradue of an Accredition Council for Pharmacy Educion (ACPE) accredited College or School of Pharmacy with a baccalauree degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. In addition to the basic requirements the following criteria must be met: Experience. One (1) full year of creditable experience the next lower level (GS-11) or equivalent, directly reled to the Clinical Pharmacy position which handles routine medicion-reled activities in accordance with local, Veterans Integred Service Network (VISN), and nional policies and regulions. These include, but are not limited to: reviewing, interpreting, and verifying medicion orders for approprieness; processing and filling medicion orders; interacting with and making recommendions to other clinical staff regarding medicion therapy ordered to ensure safe and effective care; reviewing the pient's medicions, allergies, labs, and other pertinent informion from the medical record to identify and solve medicion-reled problems; contacting providers as approprie; documenting recommendions and interventions; providing refill extensions and partial medicion supplies; taking health and medicion histories; performing medicion reconciliion; providing drug informion; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medicion usage evaluions; documenting and assessing adverse drug events (ADEs); providing oversight of technical staff in all aspects of medicion distribution. Pharmacists assigned to this position must demonstre the following knowledge, skills and abilities (KSAs): Knowledge of professional pharmacy practice. Ability to communice orally and in writing to both pients and health care staff. Knowledge of laws, regulions, and accredition standards reled to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretion of laborory and other diagnostic parameters.Preferred Experience:
Experience working in outpient setting or health system pharmacy as an intern, resident, or pharmacist.References:
