Phoenix, AZ Job Details Full-time 12 hours ago Benefits Health insurance Dental insurance 401(k) Paid time off Vision insurance Life insurance Qualifications Bilingual Spanish Clinical pharmacy experience English IV infusion Licensed Pharmacist Medical assisting Full Job Description The Clinical Pharmacy Coordinator supports and collaborates with the clinical pharmacist, to ensure patients receive high-quality care related to dispensing medications at Mountain Park Health Center (MPHC). Essential Functions Room patients and schedule patients on-demand. Limit distractions for pharmacists while they are in the room with patients. Provide support for clinical pharmacy visits including but not limited to the following: Administer vaccines Filtering out medication refills Keeping track of incoming vaccines Review medication patient calls Call pharmacy for patient medication profile if needed. Request labs (INR). Schedule/Reschedule clinic appointments for patients. Schedule disease state management appointments and educate patient on what to bring to appointments.
Referrals:
Check and schedule patients.
Prior Authorizations:
Screen Prior Authorizations. Prevent duplicate requests. OTC's - Inform patient to purchase OTC. Call pharmacy for patient related activities and prevent delay in care.
Refills:
Help educate patients on how to order refills.
Faxed Refills:
follow refill protocol.
Administrative:
Make copies of AC clinic dosing cards and patient education materials and submit inventory orders for supplies as needed. Make copies of patient blood glucose log or medication schedule. Attend and participate in clinical pharmacy meetings or other meetings as assigned. Travel to multiple locations during shift. Consistent, reliable in-person attendance is required. Other duties as assigned.

MA Certification Preferred Qualifications:
Bilingual in English and Spanish Medication knowledge
