The Population Health Pharmacy Manager provides strategic and operational leadership for pharmacy-driven population health initiatives. This role focuses on improving medication-related outcomes, optimizing total cost of care, and supporting value-based care goals through data-driven medication management, interdisciplinary collaboration, and evidence-based pharmacy programs. The manager works closely with population health, care management, quality, and provider teams to improve clinical outcomes, adherence, and patient experience across defined populations.
MINIMUM QUALIFICATIONS

LICENSURE

Must additionally hold current Pharmacist license issued by state of residence.
EXPERIENCE
1. Five (5) years of pharmacist experience. 2. Three (3) years of practice as a clinical pharmacist. 3. Two (2) years of administrative or leadership experience OR Two (2) years of experience with population health or managed care from a pharmacy perspective.
PREFERRED QUALIFICATIONS
EDUCATION, CERTIFICATION, AND/OR
LICENSURE
1. Doctorate of Pharmacy, PharmD. 2. Master of Business Administration, Master's of Health Administration, Master of Science in Health-System Pharmacy Administration, or Master in Public Health. 3. Pharmacy Specialty Certification through the Board of Pharmacy Specialties (BPS), Board Certified Geriatric Pharmacist (BCGP) through the Board of Pharmacy Specialties, or Board Certified Pharmacotherapy Specialist (BCPS) through Board of Pharmacy Specialties. 4. Three (3) years experience working with value-based care models, quality metrics, or risk-based contracts. 5. Three (3) years experience working with
HEDIS, STAR
ratings, and CMS quality programs. 6. Completion of a health-system pharmacy administration residency program OR Completion of a non-health-system pharmacy administration residency program.
CORE DUTIES AND RESPONSIBILITIES

Lead the design, implementation, and evaluation of pharmacy-led population health programs (e.g., medication therapy management, adherence initiatives, high-risk medication reviews). 2. Align pharmacy strategies with organizational population health, quality, and value-based care objectives. 3. Partner with the WVU Medicine Pharmacy Services and Peak Pharmacy Services to drive cost saving strategies through sound policy and workflow management. 4. Support performance improvement in value-based contracts, including Medicare Advantage, ACOs, and other risk-based arrangements by pharmacy management through payer relationships. 5. Develop and oversee clinical pharmacy interventions targeting chronic disease management (e.g., diabetes, hypertension, COPD, heart failure). 6. Identify and reduce medication-related gaps in care, adverse drug events, and polypharmacy risks. 7. Support quality measure performance (e.g., HEDIS, STAR ratings, CMS quality measures). 8. Utilize pharmacy, claims, and clinical data to identify high-risk patients and medication utilization trends. 9. Develop KPIs and Monitor program outcomes, medication adherence, utilization, and cost metrics. 10. Prepare reports and dashboards to demonstrate pharmacy impact on population health and value-based performance. 11. Collaborate with physicians, advanced practice providers, care managers, nurses, and social workers to integrate pharmacy services into care models. 12. Serve as a pharmacy subject matter expert for population health committees, quality teams, and leadership. 13. Support provider education related to evidence-based prescribing and formulary optimization. 14. Manage and mentor pharmacy team members supporting population health initiatives. 15. Ensure compliance with regulatory, accreditation, and payer requirements. 16. Develops and implements processes, within assigned teams, through orientation, training, and education to ensure that the competence of all staff members is assessed, maintained, improved, and demonstrated throughout their employment. 17. Directs the activities of staff, within assigned teams, and monitors performance, clarifies work expectations and promotes cooperation among individuals and groups. 18. Directly supervises staff practicing in areas of primary responsibility. 19. Performs personnel functions such as interviewing, hiring and performance management of direct-report employees. 20. Assures the development of policies, procedures and other orientation and training materials to support clinical and operational services. 21. Assures adequate and timely submission of data and reports for performance improvement activities. 22. Establishes systems to encourage effective communication and working relationships between pharmacy employees and other healthcare professionals 23. Takes a proactive and innovative approach to problem identification and resolution. 24. Maintains professional knowledge and skills by active participation in continuing education activities.
