Summary:
The Clinical Pharmacy Specialist will elevate and advance the professional practice of pharmacy through collaboration, the use of evidence-based practice, cultivation of knowledge, and facilitating and encouraging career-long learning and inquiry at Penn Home Infusion Therapy (PHIT). They will be responsible for utilizing professional skills and clinical expertise to provide pharmacy training and education to staff at PHIT, maintaining equipment databases (ie infusion pump libraries) and eMR system changes. They will ensure that all education programs are in alignment with regulatory standards including, but not limited to local, state and federal pharmacy law, USP and USP 800, NIOSH, the Joint Commission, FDA, CDC, medication use system and OSHA. They will function as the clinical pharmacy and home infusion expert in creation of policies, standard operating procedures, pump programming and documentation. In addition, he/she partners with other Penn Home Infusion Therapy departments and other Penn Medicine at Home leaders in building strong relationships to meet their needs. The Clinical Pharmacy Specialist performs various other education and training duties related to pharmacy and patient care as necessary. Responsibilities:
In collaboration with the PHIT Management team and other key stakeholders, develops, implements, and evaluates designated programs including but not limited to: orientation, required annual, continuous competence assessment, continuing education for pharmacist and pharmacy technicians, as well as vendor in-services. Coordinates entry-level orientation, education/training and competence assessment for existing pharmacy staff, new staff, employees with role changes, and supervisory personnel. Creates new workflows/SOPs for Clinical Pharmacy teams with any eMR changes, clinical guideline updates, or regulatory changes Act as the pharmacy informatics representative, making changes in CPR+, and working with the PennChart team as necessary Maintain and update smart pump library to adhere to new therapy additions, therapeutic guideline changes, or specific PHIT or patient needs Maintain current knowledge in the relative areas to address the constant changes, address competency and performance gaps, and evaluate and recommend viable and sustainable solutions. Demonstrates the ability to adapt educational goals to the learning audience to ensure success Assists in coordination of annual/biennial USP and USP 800 competencies for all pharmacy staff. Review and educate all PHIT staff on new therapies to market Credentials:
Pharmacist (Required) PA Pharmacist license in good standing required. Education or Equivalent Experience:
