Job Description
What You Will Do:
Ambulatory Pharmacy coordinator working within a multidisciplinary team with a primary focus on Anticoagulation but also extends to non-anticoagulation ambulatory work with Primary Care, Heart Failure, Population health and Pulmonary disease state management. Collaborates with clinical teams to manage and recommend treatment plans for patients within the ambulatory setting. Oversees clinic projects and initiatives within the ambulatory setting including their creation, design, and implementation. Educates and precepts pharmacist learners including Advanced practice pharmacy experiences as well as resident pharmacists.
Requirements:
Bachelor's degree in Pharmacy Pharmacist (R.Ph. or RPh) Basic Life Support Certification (BLS) required ACLS and PALS certification preferred WI Driver's License Hold and maintain BCPS (Board) certification in pharmacotherapy. 4 years of relevant experience preferred
