Job Description

Summary This role is responsible for assisting health plan members address medication related problems and preventative care needs through outreach, education, and problem-solving. In this role, you will work with members, pharmacies, and healthcare providers to identify and close healthcare gaps. Success in the role requires strong communications skills, some clinical knowledge, attention to detail, and critical thinking when resolving member concerns. This position is ideal for someone with pharmacy experience who enjoys helping people, building relationships, making a meaningful impact through member/patient outreach and support.
  • The ability to communicate effectively in both English and Spanish is required for this position Primary Responsibilities
  • Make proactive outreach to English and Spanish speaking health plan members, pharmacies, and healthcare providers to identify and close clinical care gaps in support of Medicare Stars and HEDIS quality measures.
  • Analyze barriers to care such as transportation problems, medication adherence issues, and pharmacy claim rejects. Take appropriate steps to resolve care gaps or coordinate solutions.
  • Educate and support members regarding pharmacy benefits, medications, and clinical guidelines to help close care gaps.
  • Respond to pharmacy and benefit-related inquiries from members and health plan staff, providing accurate information or escalating complex issues as appropriate.
  • Research and resolve pharmacy claims issues, including troubleshooting adjudication problems and coordinating corrections with internal teams or external partners.
  • Record outreach activity and document member interactions to support care gap monitoring and improvement.
  • Collaborate with internal teams and leadership to ensure timely resolution of issues impacting quality measures.
  • Perform other duties as assigned to support team, organization, or program needs. Benefits & Schedule

Up to $24.72 per hour

Experience:
retail pharmacy: 1 year (Preferred) PBM or health plan: 1 year (Preferred) Medicare Stars or other pharmacy/clinical quality initiative: 1 year (Preferred) member/patient oureach: 1 year (Preferred)
