Polaris Transitional and Extended Care Center in Anchorage, AK, is a skilled nursing facility with long term care, post-acute rehab, and geriatric therapy services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program! Pay Range $50.00•$65.00 hour based on experience

Flexible schedule with work-life balance Call or text Paul Medvene for more information at 949-230-8384 or e-mail We are high paced facilities with both short-term and long-term care: The extended care facility is inspired by the Green House care model, offering top-tier skilled nursing and rehabilitation services within a homely ambiance for its residents. 96 beds spread out over 8 free standing cottages each private room converges around a spacious central area, boasting an open-plan kitchen and dining space. This design prioritizes social interactions and nurtures a cozy atmosphere. The transitional care facility is a 50-bed high tech facility with all individual rooms and a therapy gym with all the high-tech equipment and space a passionate therapist could ask for. Anchorage, Alaska, is a beautiful city and is the gateway to many outdoor adventures. Anchorage boasts a diverse population including Alaska Native people from various tribes and cultures. This is a perfect location for your exciting lifestyle.
Duties:
Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards. Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements. Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care. Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders. Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
Qualifications:
Physical Therapist license is required. Open to Physical Therapists at all experience levels. New grads are welcome!
