3 + months, with potential for extensionPay Range:
$57 to $59/hrLocation:
Coverage areas: Fort Smith, AR and surrounding areaSchedule:
8 am to 4:30 pm; Monday - Friday- Mileage is reimbursed between visits, from first patient to last patient.
Travel Physical Therapist:
- Evaluate patient functional status, body systems, and physical therapy needs in accordance with CMS requirements; collaborate with physicians to develop care plans.
- Provide physical therapy services within state practice guidelines, plan of care, and interdisciplinary care team coordination.
- Monitor patient progress, document responses to treatment, and report condition changes to the nurse supervisor and/or physician.
- Deliver evidence-based treatment to relieve pain, restore function, and maximize patient independence and mobility.
- Educate patients, families/caregivers, and care team members on treatment plans, functional limitations, and progress toward goals.
- Complete patient evaluations, develop care plans, andmunicate evaluation oues, goals, and care frequency within required timelines.
- Maintain accurate and timely documentation, including progress notes, discharge summaries, and required reports per agency policy.
- Travel throughout the assigned service area and support nearby agency locations as needed.
- Supervise Physical Therapist Assistants (PTAs) and home health aides as permitted by state regulations and agency policy.
- Participate in staff meetings,mittees, and agency-requested education or training activities.
- Maintain current clinical knowledge, licensure, andpliance with therapy standards of practice.
- Maintain schedule flexibility, including weekend rotations and potential holiday coverage, to support patient care needs.
- Perform additional duties as assigned.
Travel Physical Therapist:
- Home health and Oasis experience required
- Physical Therapist license in Arkansas
