Job Description
Job Summary We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, develop, and implement personalized treatment plans to improve patients' mobility, reduce pain, and enhance overall quality of life. Your expertise will support a wide range of patient needs across inpatient, outpatient, home health, and community care settings. The ideal candidate is passionate about patient care, possesses strong physiology and anatomy knowledge, and thrives in a collaborative environment focused on recovery and well-being. Responsibilities Conduct comprehensive patient evaluations to determine physical therapy needs using medical terminology and anatomy knowledge. Develop individualized care plans targeting specific conditions such as orthopedics, geriatrics, pediatrics, or neurological impairments. Implement therapeutic interventions including physiotherapy techniques, trigger point therapy, and other modalities to promote healing and mobility. Monitor patient progress regularly and adjust treatment strategies accordingly to ensure optimal outcomes. Coordinate discharge planning and collaborate with healthcare teams to facilitate seamless transitions from hospital to home or community care settings. Maintain accurate documentation of patient care activities within electronic medical record (EMR) systems. Educate patients and caregivers on exercises, injury prevention, and home-based therapies to support ongoing recovery. Requirements Valid licensure as a licensed Physical Therapist with relevant state or national regulatory bodies. Proven experience in inpatient hospital settings, outpatient clinics, or home health environments. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy. Experience with discharge planning, care plans development, and working within EMR systems. Familiarity with acute care protocols, orthopedics, geriatrics, pediatrics, and community-based rehabilitation services. Excellent communication skills for patient education and interdisciplinary collaboration. Ability to work effectively in fast-paced environments such as hospitals or outpatient clinics while maintaining attention to detail.
