Physical Therapist For Our Independence PT (FOI) Glendale, AZ Job Details Full-time $35 - $50 an hour 15 hours ago Qualifications Collaborate with healthcare professionals Anatomy knowledge Inpatient Patient assessment Manual therapy Outpatient Patient interaction Orthopedics Medical terminology Full Job Description Job Overview Join our dynamic healthcare team as a dedicated Physical Therapist, where your expertise will empower patients to regain mobility, reduce pain, and improve their overall quality of life. In this vital role, you will assess individual needs, develop personalized care plans, and deliver evidence-based physiotherapy interventions in the ALF setting. Duties Conduct PT evaluations on easy to use WebPT EMR Deliver hands-on therapy sessions in the ALF setting at fabricated wellness centers or homes of the client allowing functional application. Manage referral/discharge planning processes by coordinating with multidisciplinary teams to ensure health and wellness at home Monitor patient progress regularly, adjusting treatment strategies based on physiology responses and clinical observations. Integrate with ALF wellness teams/providers with focus on health/wellness and patient outcomes. Qualifications Valid Arizona licensure or certification as a licensed Physical Therapist Strong knowledge of physiology, anatomy, medical terminology, and specialized manual therapies. Demonstrated ability to develop effective care plans for vulnerable ALF population. Excellent communication skills to collaborate with healthcare teams and educate patients effectively.
