Responsibilities:

Complete initial home health assessments for patients and families Conduct in-home visits, assessing progress toward goals Develop personalized care plans and incorporate therapeutic/preventive actions Counsel and educate patients and their families Supervise PTAs (if assigned to the case)

Requirements:

Active AZ PT License (required) Minimum 1 year of Home Health PT experience (required)

OASIS-E SOC

documentation experience (required)

AXXESS EMR

experience preferred Reliable transportation, valid AZ driver's license & auto insurance
