Key Responsibilities Assess and evaluate patients' therapeutic, rehabilitative, and functional needs. Develop and implement individualized care plans tailored to each patient's condition. Evaluate home environments for safety and recommend modifications as needed. Provide direct physical therapy treatments and educate family members or caregivers. Identify additional home health services needed and make appropriate referrals. Prepare and submit clinical progress reports based on goal achievement. Offer in-service education for nursing staff as necessary. Supervise Physical Therapy Assistants in accordance with policies and state regulations.Qualifications Education:
Bachelor's degree in Physical Therapy or higher from an accredited institution.Licensure:
Active Physical Therapist license in Arizona.Experience:
At least one year of relevant experience; community/home health experience preferred.Language Skills:
Bilingual (English/Spanish) strongly preferred.Skills:
Strong verbal and written communication skills with excellent organizational abilities.Certifications:
