MINIMUM REQUIREMENTS
Arizona Physical Therapy License Verbal & Written communication skills in English and a demonstrated ability to read and comprehend written/graphic and oral instruction Must release Social Security Number to be eligible for AHCCCS-Provider ID Number
REQUIREMENTS PRIOR TO START DATE
Arizona IVP fingerprint clearance card Proof of immunity to rubeola (measles) and rubella (German measles), or proof of MMR immunization Valid Driver's license (Arizona Driver's License required within 30 days of hire) MVReport
