The ideal candidate will possess a passion for improving patient outcomes within the older adult population. This role involves assessing patients' needs, developing individualized treatment plans, and providing therapeutic interventions to enhance mobility and quality of life. The Physical Therapist will work closely with patients of various age groups, including geriatrics, and collaborate with other healthcare professionals to ensure comprehensive care. Responsibilities Conduct thorough assessments of patients to determine their physical therapy needs. Develop and implement personalized treatment plans based on individual patient goals. Utilize various therapeutic techniques and modalities to facilitate recovery and improve function. Educate patients and their families on treatment plans, exercises, and self-management strategies. Monitor patient progress and adjust treatment plans as necessary. Maintain accurate documentation of patient care, including evaluations, progress notes, and discharge planning. Collaborate with interdisciplinary teams to coordinate comprehensive patient care. Benefits
